question set on market research for INSURANCE

i am doing a project on market research of customers satisfaction in bajaj allianz, can anybody help me out on what kind of questions i should prepare for this.

I think for this project
you should follow the following pattern
1. set the objective
2. based on each of the objective a set of question will be required to be prepared say for example
My objectives are
1. Understand the brand awareness of bajaj allianz
then the question would be
a. Have you ever invested in insurance Yes / No
b. If yes then pl name three companies in insurance sector that comes to your mind
c. If bajaj allianz is there, then which products does bajaj allianz offer
list
d. Have you bought any of its products
if yes
and so on.............................
