As for the second question, with the perspective of international level, remember one thing and that is the client on the other side receives an n number of call per day. so most of the times for him it just happens to be another sales call.
To be successful :
1. understand your product and as to what makes you feel that the client may be interested in your product.
2. study the clients company profile and workforce strength (very imp). also the profile of the person you wish to speak to ( that is the target audience (VP, VP Sales, CIO, CFO)
3. lot more other stuff but this might help a bit
